Report Issues or Feedback with the Phaze Platforms

This form is for users of the Phaze app or the Phaze Coaching Hub to submit issues and feedback. Check out our Frequently Asked Questions before submitting.

Please make separate form submissions for each distinct issue you're facing.

First, choose the Phaze platform you want to submit a report for.

App version *
Please specify which version of the app you're running. You can see the version at the bottom of the app's account screen.
For example, "0.0.6"

Device Make, Model and OS version *
Please describe the device you're using the app on and its operating system version. For example, "Apple iPhone 14 Pro with iOS 16" or "Samsung Galaxy S22 with Android 12"

Report Type *

Issues are instances when the app is not working as expected. Please provide as much information as possible - it will make it easier for us to fix the issue!

Description of the Issue *
Please describe the issue you are facing, including: 
• If your issue is with a specific trip - please provide the trip name, as well as the key event (e.g. seek light, avoid light etc.) and the date (e.g. 25 Feb) if applicable.
• When did the issue occur? Did it happen once or is it a recurring problem? 
• What was the expected behaviour of the app? What happened instead? 

Please describe what happened in your own words. The more detailed you are, the easier it will be for us to resolve the issue.

Steps to reproduce the issue *
A step-by-step account of what you were doing when the issue occurred. For example: 
1) Opened the app
2) Created a trip 
3) ...

Issues are instances when the Coaching Hub is not working as expected. Please provide as much information as possible - it will make it easier for us to fix the issue!

Description of the Issue *
Please describe the issue you are facing, including:
• Please provide the name of the app user, the trip name or the specific metric experiencing the issue, if applicable.
• What was the expected behaviour of the Coaching Hub? What happened instead?

Please describe what happened in your own words. The more detailed you are, the easier it will be for us to resolve the issue.

A step-by-step account of what you were doing when the issue occurred. For example:
1) Logged into the Coaching Hub
